I am referring to a present situation. My sentence is, “Also, she ask that we continue to keep her in our prayers.” Is ask correct or should it be asks?

It should be ‘asks’ because you need a third person singular form of the verb (ask) to go with the third person singular pronoun ‘she.’ And even if the explanation doesn’t make sense, at least you were on the right track.
