Where would a comma go in the following sentence? Each year five thousand people die waiting for their “wish” to come true.

It is optional to put a comma after an introductory phrase of less than five words. Yours is two. However, you have another grammatical issue. You have a plural subject (five thousand people) and you use the plural pronoun correctly (their), but I doubt all these people are sharing one wish. Consequently, wish needs to be wishes.
